The Position: The Nurse Practitioner (NP) in this role will deliver comprehensive, person-centered primary and palliative care services to seniors through home-based clinical settings. The NP will focus on quality of life, chronic disease management, and health promotion—aligned with the mission of Snowline Health and the needs of our community. As part of a multidisciplinary care team, the provider will coordinate care, develop robust treatment plans, and guide complex care transitions to support program participants and their families.
