Apply now and a recruiter will reach out with more details. Minimum Requirements Bachelor's, Master's, or Doctoral degree in Physical Therapy from an accredited program approved by the APTA. Active CA Physical Therapist license required to start the assignment. Minimum one year of home health Physical Therapist experience required. Experience prioritizing start-of-care visits and next-day mandatory reporting of start-of-care visits. Experience supervising Physical Therapy Assistants. Clinical documentation skills with ability to complete notes and other documentation within 48 hours of visit. Familiarity with Kinnser or similar home health documentation systems preferred. BLS certification from AHA or ARC may be required. Ability to maintain timely communication with supervisor, intake, and office and be generally responsive during normal business hours. Assignment Details Facility type: Hospital (home health role covering county territory). Shift: Days. Schedule: 40 hours per week, Saturday through Wednesday. Required priorities: start-of-care visits; next-day mandatory reporting for start-of-care visits. Documentation timeline: clinical notes due within 48 hours of visit. Training: three full days (one in-house, two in the field). Documentation system: Kinnser. Equipment issued: iPad and Verizon One Talk. Territory: Solano County, CA. Candidate must be willing to expand territory as needed. May be required to float to other facilities or service areas as needed.
