Job Description:
The Center Manager for NEMS PACE (Program of All-Inclusive Care
for the Elderly) program has the responsibility for a wide range of
disciplines and services provided through the PACE Center,
including all administrative responsibility related to direct
operation of the PACE Center. The Center Manager coordinates
overall operations of the PACE Center and serves as the liaison
between direct care staff and administration. ESSENTIAL JOB
FUNCTIONS: Coordinate all center activities including care planning
and implementation, preparation for interdisciplinary conference
and daily meetings, family conferences, and initial and period
assessments Supervise clerical staff, social workers,
transportation staff, recreational staff, dietary staff, and center
aides Facilitate and lead morning meetings of the interdisciplinary
team (IDT) Facilitate and lead regularly scheduled Care Plan
meetings to review participant cases on a regular basis Oversee the
quality of care and safety of PACE participants, both at the PACE
center and in the community Serve as a liaison between disciplines
and between administration and direct staff Work closely with the
medical care team as part of the IDT to develop and implement
participants’ individualized care plans Coordinate 24-hour care
delivery Coordinate with the marketing department to hold site
tours for new candidates at the PACE Center Serve as a
representative of NEMS PACE to the community Coordinate activities
related to new participants Manage the center’s budget Communicate
with North East Medical Services administration on a regular basis
about center operations and needs Oversee overall documentation
cohesiveness and accuracy in each participant’s medical record
Utilize Epic appropriately and efficiently and serve as content and
workflow expert for resolving issues and proposing solutions in
Epic Perform other job duties as required by supervisor ADDITIONAL
RESPONSIBILITIES: Work independently and as an effective member of
a team Lead and facilitate IDT decision making Be organized within
busy center and clinic environments Establish and maintain
effective interpersonal relationships with all levels of staff,
other departments, agencies, members, providers, and the general
public Communicate effectively, verbally and in writing, with
individuals from varying cultural backgrounds Prepare concise and
clear reports Identify pertinent policy and operational issues;
develop strategies, solutions, and recommendations Multi-task in
projects and their respective activities, timelines, and issues
Utilize computer and software tools Only act within the scope of
his/her authority to practice Meet a standardized set of
competencies for the specific position description established by
NEMS PACE and approved by the regulatory agencies before working
independently Be medically cleared for communicable diseases and
have all immunizations up-to-date before engaging in direct
participant contact Pass a criminal background check which will
verify that the candidate is: not excluded from participation in
Medicare and Medicaid programs; has not been convicted of criminal
defenses related to their involvement in Medicaid, Medicare, other
health insurance or health care programs, or social services
programs under title XX of the Act; and have not been convicted of
physical, sexual, drug, or alcohol abuse QUALIFICATIONS: Bachelor’s
degree in a health-related field required, Master’s preferred. At
least 3 years of experience in a supervisory capacity within a
healthcare setting. Minimum one year of experience working with
older adults and their families in a healthcare or community agency
setting is preferred. Must be available to work any day of the
week, Monday through Sunday, depending on need. Candidate must be
comfortable working in a dynamic environment with direct care staff
and administration staff. Excel in organizational and supervisory
skills, with an ability to effectively handle difficult situations.
Experience in successfully managing and completing projects is
required. Experience with senior care clinical programs similar to
PACE preferred. Experience working with clinical charts and
records. Knowledgeable of the physical, mental and social needs of
the elderly and be a professional in one of the following: Nursing,
Social Work, Psychology, Recreation, Occupational Therapy, Physical
Therapy, Speech Therapy, Dietetics, Gerontology Current BLS and
First Aid required LANGUAGE: Must be able to fluently speak, read
and write English Fluency in other languages are an asset STATUS:
