Job Description:
We are currently seeking a Medical Social Worker (MSW) to join
the team at Oakland, CA . The Medical Social Worker will provide
biopsychosocial assessments, crisis intervention, discharge
planning, short-term counseling, and resource referrals for
patients and families in the inpatient acute care setting. Key
Responsibilities: Conduct comprehensive biopsychosocial assessments
for patients. Provide crisis intervention and short-term counseling
to patients and their families. Develop and implement discharge
plans in collaboration with the interdisciplinary team. Coordinate
care transitions and referrals to community programs and healthcare
resources. Facilitate psychoeducation and support groups, as
appropriate. Collaborate with internal hospital teams and
community-based organizations for patient care coordination.
Document assessments, care coordination activities, discharge
plans, and all other interactions in accordance with hospital
policies. Educate staff, patients, and families on available
resources and healthcare planning. Required Qualifications:
Master’s Degree in Social Work (MSW) – Required Minimum of 2 years
of recent experience as a Medical Social Worker in an acute care
hospital setting. Charge/Lead experience required. Proven
experience in discharge planning and coordination in inpatient
environments. Experience with Epic EMR – Preferred Travel
experience required. Certifications: MSW (Master of Social Work) –
Required BLS (AHA) – Preferred but confirm during submission
Preferred Skills & Experience: Psychiatric hospital experience
51/50 holds knowledge (preferred) Familiarity with community-based
resources and programs (Medicare, Medicaid, Social Security,
Veterans programs, housing assistance, etc.) Knowledge of
protective services (Adult & Child Protective Services)
Biopsychosocial assessment expertise Case management and care
coordination experience Knowledge of high-risk interventions,
guardianship, power of attorney processes, and advance directives.
Additional Details: Patient Ratios: Approximately 1:15-30 Patient
Age Groups: Adults & Geriatrics Weekend Requirement: Every other
weekend Charting System: Epic (Preferred) Daily Census: ~160
patients Floating: Not required RTO: 1 holiday off allowed; must be
provided at time of submission Locals: Accepted at a lower rate
Background: Must disclose any hits at time of submission Compliance
Requirements: Valid California MSW license required at time of
submission 2 recent supervisory references required during
compliance Must complete modules prior to or during orientation
(average 1–5 hours, factored into NBO) Must be available to
